What is the role of the Case Manager?
The Case manager connects students that are identified as an at-risk student with resources that could assist them in maintaining optimal mental health and general health and well-being. The goal is to provide support and assistance to student so that they can reach their full academic and social potential.
A case manager can…
- Perform an assessment of the individual student
- Plan in conjunction with the CARE (Campus Assessment, Response, and Education) Team and student
- Coordinate services for students
- Evaluate and provide follow up with student
- Connect services with the student’s “unique needs”
